DAY SERVICES CASE MANAGER

Department: Adult Day Activity Program & Training (ADAPT)

Hours: Full-Time - Monday to Friday

Description of Position: Responsible for coordinating agency services that enable an individual with intellectual disabilities to receive a full range of appropriate services in a planned, coordinated, efficient and effective manner to promote the well-being of the individual. Responsible for locating, managing, coordinating, and monitoring all proposed services. Responsible for attending case conferences/meetings and making recommendations for needed medical, social, educational, and informal community supports needed by eligible person within the Day Services Program at TradeWinds. Responsible for oversight of funding resources to assure they are adequate for planned services provided by TradeWinds. Must be available Monday – Friday, 7:30am – 4:00pm.

Education/Certification Requirements: Bachelor’s Degree preferred.

Experience Required:  One year of experience working directly with persons with intellectual disabilities. Prior experience in developing and implementing individual programs, program evaluation, and in working with a team is desired.

Position Reports to:   Lead Case Manager

Additional: Must have a valid driver’s license with no suspensions for traffic infractions during the past three years and be at least 18 years of age. Must clear TradeWinds background checks.
